In agreement with all stated rules and procedures for use of the Cornerstone Nashville Playgrounds, I hereby give consent for my child to participate in the activities of the playgrounds at his/her own risk. I understand and agree that Cornerstone Nashville is not liable for injuries of any kind or degree that may occur during the use of the playgrounds, and I hereby release and discharge from liability of any claim or lawsuit that I may have for myself and for my minor child/children the church, its directors, officers, employees, volunteers and agents. I agree that it is my responsibility to provide accident insurance for my child. My agreement indicates that I have read and understand the rules of the Cornerstone Nashville Playgrounds, and that I am willing to accept full responsibility for my child while participating in Playground activities.
